lone fieldsets which should belong to the accending fieldset have been (mostly) merged. Some outstanding issues with the permissions area (Dominik has some stuff for this at some point anyway) and the Custom Profile Fields bit, which should feature "Previous/Next" style buttons.

re-introduce append_sid: old style continues to work, not a performance hog as it was in 2.0.x -> structure is different
apart from this, code cleanage, bug fixing, etc.
